Bier blocks were IV regional blocks first described by German surgeon August Bier in 1908 involving two tourniquets and procaine. It is excellent for procedures less than 90 minutes in duration. An IV is placed distally in the extremity. Two tourniquets are placed. After exsanguination of the extremity the proximal tourniquet is inflated and a total dose of lidocaine (4 mg/kg) is injected. When the patient complains of pain the distal tourniquet is inflated and the proximal tourniquet is deflated.
