4.  Review the present chemotherapy protocol which is now used.  Also review the present RT protocol now used.  Give the side effects and incidents of complications.  Who is a candidate for bone marrow transplant?

Answer:
No regimen has been shown to be more effective than CMF [cyclophosphamide, methotrexate, and 5-FU]. CA [Cyclophosphamide with doxorubicin (or Adriamycin)] is often used. Both regimens cause nausea, vomiting, myelosuppression, and thrombocytopenia. The optimum treatment regimens are 3 to 4 months. Tamoxifen should be administered for at least 2 years. It is common practice to excise the tumor, administer chemotherapy for 4 to 6 months before administering radiotherapy. Chest wall doses range from 45 Gy to 50.4 Gy in 2 Gy fractions. The incidence of locoregional relapse with and without radiation is 7% vs. 31%. An overall survival benefit is seen in those receiving radiation, with a mean overall survival of 50 months vs. 20 months, and a 3-year overall survival of 88% vs. 46% with and without radiation.
