In 1990, Benelli described an approach in which a concentric, oval area surrounding the areola is deepithelialized.  The dimensions of this egg-shaped region correspond to the diameter of the new areola. The breast skin is undermined only at the lower pole, leaving the nipple-areola on a wide, superiorly based dermoparenchymal pedicle. The large glandular flap beneath the areola is split vertically and each wing is tacked to the 4th or 5th rib periosteum on the opposite side of the breast, in effect creating a criss-cross parenchymal sling periosteal mastopexy.  A non-absorbable purse-string suture is placed at the level of the dermis around the outer perimeter of the incision. Tightening the suture narrows the diameter of the periareolar skin. Any excess can be resected at the time of closure.
