In the 1976 PRS article entitled "Secondary correction of the too-high areola and nipple after a mammoplasty" by R. Millard, W. Mullin, and M. Lesavoy, two general approaches to correction of this problem are proposed: 1) repositioning of the areola-nipple component or putting in an implant to make the main projection of the breast directly behind the nipple.  2) adjusting the relative position by shortening the distance from the inframammary line to the areola.   See the paper for images.
