13.  What postop routine is used for a cleft lip patient?

Answer:
Postoperative management is geared towards minimizing tension and avoiding disruption of the repair.  Bottle feeding is forbidden for the first several days since the trauma can be inflicted by the bottle's nipple and plastic ring as well as by the action of sucking itself.  Instead, the patient can be fed using a cup or a syringe attached to a short piece of soft rubber or plastic tubing (a Breck feeder).  The arms are placed in elbow immobilizers to keep the patient from manually or digitally disturbing the repair.  About 5-10 days post-op, the patient can begin regular nipple feedings and the immobilizers are removed.
