2.  What are upcoding, unbundling, and exploding?

Answer:
Upcoding refers to coding for a more extensive or complicated procedure or service than was actually done.  Unbundling refers to submitting multiple codes for procedures that are more accurately described by a single code.  Exploding is from the Latin explodere which means to drive off the stage by clapping.  Alternatively, it means showing the parts separated but in correct relationship to each other (e.g. an exploded view of a carburetor).  With regard to coding, exploding may just be the same thing as unbundling.  Whatever it is, don't do it.
