18. Your first blepharoplasty patient cannot see in the recovery room.  What do you do?  Is blindness common after blepharoplasty?  What is its cause if present?

Answer:
Before calling your lawyer or malpractice insurance agent, you should cut all sutures and open the orbital septum widely to release pressure caused by a presumed retrobulbar hemorrhage.  Reduce or stop completely any intravenous fluids and give intravenous mannitol to help reduce extracellular fluid volume and hinder edema development.  The wound should be explored for any bleeding points.  If retrobulbar pressure does not appear to be relieved at this point, then lateral canthotomy should be performed. The exact incidence of blindness is not known but it is considered to be exceedingly rare.  Blindness will usually be preceded by progressively worsening pain and proptosis with deteriorating vision and a dilated fixed pupil.
