4.  What solution of steroids should you inject into a keloid?  How many injections are needed?  Does it work?

Answer:
Triamcinolone acetonide is best used in a concentration of 40 mg per ml; smaller concentrations are ineffective. (McCarthy)

According to a 1991 paper, the optimal treatment for keloids remains undefined. A randomized, prospective study was performed to compare two treatment regimens that were believed to be promising. One regimen consisted of surgery plus two preoperative and three postoperative intralesional injections of triamcinolone (27 keloids in 16 pts); the other regimen consisted of surgery plus oral colchicine (1.2 mg per day) initiated three weeks after surgery and continued for 5 months (28 keloids in 20 pts). There were no significant differences between the two groups in terms of sex, race, age, keloid location , keloid size, the duration the keloid was present, and the percentage of keloids that were recurrent.  Only 30% of keloids treated by the first method were cured, and 32% of keloids treated by the second method, so that no significant differences between the two regimens were noted.
