27.  Discuss endoscopic abdominoplasty.  (PRS 1997; 99:895)

Answer:
Endoscopic abdominoplasty involves making a small suprapubic transverse incision and raising cutaneous flaps up to the level of the umbilicus. The rectus is then plicated endoscopically. Suction lipectomy is used if indicated. Only a small percentage of patients are candidates for endoscopic abdominoplasty. The procedure is useful for patients with a contour deformity limited to the lower abdomen with minimal skin excess. Patients have musculofascial laxity that can be elicited by having the patient bend over at the waist to relax the muscles and should worsen the musculofascial laxity. This is the so called “divers” test by Gremminger.
