The article reviews 2000 pressure sores in paraplegics at the VA in the Bronx from 1950 to 1962.  States that recurrences can be avoided by adhering to the following principles: 1) excision of the ulcer, scar, soft tissue calcification and bony prominence, 2) use of flap of muscle and fascia to cover bony stump and to fill dead space, 3) coverage with large regional pedicle flap and skin and fat, and 4) use of thick split thickness skin graft to cover the donor site of the flap.  Ischial, sacral and trochanteric ulcers were most common and the most difficult to treat.  Success rates “following exacting and complete surgery” were 95, 85 and 86 percent, respectively.
