7.  The concology service calls you.  They have a patient who just had an adriamycin extravasation.  What should you do?

Answer:
Chemotherapy extravasation injuries are best treated acutely by rest, elevation, a splint, and local ice application.  Calcium gluconate has been used as well.  Some recommend ice application for 48 hours, the administering sulfadiazine to prevent secondary infection, and allowing the tissue to demarcate before excising the necrotic tissue.  Closure can then be achieved with whatever method is necessary - delayed primary, skin grafting, local flaps, or distal flaps.
