Answer:
A 1988 PRS pig study showed that flaps raised with expanders were more hardy compared to simple delay flaps (diameter of axial vessel was larger than control) and theorized that keeping the capsule was beneficial because it enables the flap to be raised quickly on removal of the expander.  It also supplied a well-vascularized but non-bleeding backing.
